Marker of J.E. and A.D. Ash

The most interesting thing about this marker is that A.D. may or may not be buried here.  A.D.’s  death certificate states he was buried on 31 December 1922 in Bear Springs Cemetery.  See the death certificate below:

 

What do you think happened here?  I can think of only three possibilities: 1- The death certificate is wrong; 2- He was moved; 3 – The marker is a cenotaph.  I may never know for sure.
